For new clients, please click on the link below for the Initial Intake Forms, and complete the required HIPAA compliant packet prior to your scheduled therapy evaluation. We've worked to make sure this is a simple process that does not require printing, scanning, using non-HIPAA compliant e-mail methods, or delivering hard-copy forms to the clinic. When you complete the required sections, you simply click submit on the last form. Please attach or email any previous evaluations and assessments, and submit a copy of your insurance card. Update to Cancellation Policy: I am unable to hold scheduled session times for anyone who misses more than two weeks in a row, or in the event you have two or more no-shows, have more than two cancellations within the four-hour session window, or cancel more than 25% of scheduled sessions.
